You'll need to fill out the below SF-1187.  When filling in the SSAN, use 000-00 for the first 5 digits.  Ignore the timekeeper number.  Add your personal e-mail and personal phone at the bottom of the address block if you want the Union to have this information.  Sign your form personally and scan it in, or use your PIV card to sign it with a PIN.  Finance will frequently reject forms that do not have an actual signature or a PIV signature.  NOTE: To be eligible for membership, box 37 of your most recent SF-50 must read "0054" which is our bargainig unit code.
